Medication Policy The school MD has given standing orders for basic medications that can be given at the discretion of the nurse unless otherwise stated by parents. These include but are not exclusive to Tums, Visine, Chloraseptic Spray, Orajel, Cough Drops, Benadryl Cream, Hydrocortisone Cream. Children are not allowed to carry ANY type of medicine with them throughtout the day unless special permission has been granted by the Health Office.
Short Term Medications (1 week or less)
Whenever a student requires medication at school, the medicine must be brought directly to the nurse at the beginning of the day. Only medication in its original pharmacy container which states the name and telephone number of the pharmacy, the prescription number, the student's name, the name and dosage of the medication, directions and administering and the name of the physician will be accepted. Prescription medication also requires written permission from the parent.
Long-Term Medication ( Daily, PRN, or greater than 1 week)
The School Nurse may administer any form of medication that is in the original container and is accompanied by the District Medication Form. completed and signed by the child's doctor and parent/guardian. Forms can be obtained through your School Nurse, the main Nurse page, Dsitrict web site, or you can contact the Health Office at 732-297-2488 ext. 5603.
****If your child should suffer from asthmaor allergies that require medications to be kept in school, please contact the Health Office for further instructions.****

Immunization PolicyImmunizations are required for entry into school to comply with Chapter 14 of the New Jersey Sanitary Code. Failure to provide a complete immunization record can result in the child's exclusion from school. The schools nurse reviews all immunization records prior to a student's admission. The following immunizations are required in order for a student to enter school:
DTaP (Diptheria, Tetanus, and Acellular Pertus
Every child less than seven years of age shall have received a minimum of four doses of diptheria and tetanus toxoids and pertussis vaccine (DTP), or any vaccine combination containing DTP, such DTP/Hib or DTaP, one dose of which shall have been given on or after the child's fourth birthday.
IPV (Polio)
Every child less than seven years of age shall have received at least three doses of live, trivalent, oral poliovirus vaccine (OPV), or inactivated poliovirus vaccine (IPV), either separately or in combination, one dose of which shall have been given on or after the child's fourth birthday or, alternatively, any appropriately spaced combination of four doses.
For children seven years of age and older, any appropriately spaced combination of three doses of OPV or IPV shall satisfy the poliovirus vaccine requirement.
MMR (Measles, Mumps, and Rubella)
Every child born on or after January 1, 1990 shall have received two doses of a live measles-containing vaccine, or any vaccine combination containing live measles vaccine, such as the preferred measles, mumps, rubella (MMR) vaccine, prior to school entrance for the first time into Kindergarten, Grade One, or a comparable age entry level special education program with an unassigned grade. The first dose shall have been administered on or after the child's first birthday, and the second does shall have been administered no less than one month after the first dose.
HepB (Hepatitis B)
Every child born on or after January 1, 1996 shall have received three doses of hepatitis B vaccine, or any vaccine combination containing hepatitis B virus, prior to school entrance for the first time into Kindergarten, Grade 1, or a comparable age entry level special education program with an unassigned grade.
Varivax (Varicella or Chicken Pox)
Every child born after 1998 is required to have received one dose of varicella vaccine, prior to entering the school system, including pre-school. Vaccinations must be administered on/after the child's first birthday or parental/physician's history of disease or lab evidence of immunity.

Physical EducationA child may be excused from physical activity for three consecutive days with a note from a parent/guardian. If the time is extended beyond three days, a note from the family physician is required.
If your child cannot partake in physical education, then he/she will NOT be permiited to participate in outdoor recess.
If your child is using some sort of apparatus, including but not limited to a cast, sling, ace bandage, and/or crutches, he/she will not be able to participate and will require a note from a physician to resume physical activity.
Please remember to provide your child with appropriate footwear, such as sneakers that tie or have Velcro. Clogs, sandals, boots, or party shoes are not permitted.

School Enrollment and Transfer InformationThe following information must be provided prior to your child entering school:
• Physical examination by a licensed health care provider and a completed Physical Exam Form.
• Evidence of completed immunization requirements (see above).
• Completed health history form (go to South Brunswick District Health Forms page).
